Comparison of creatinine clearance in urine 24-hour volume and renal function estimation from the MDRD formula in patients with Chronic Kidney Disease. Experience of Susalud

Abstract

Objective: to compare the results of creatinine clearance in the 24-hour urine output with the results derived from the MDRD formula.


Methods: analytical study of diagnostic tests. Information was taken from the medical records of 1 409 patients aged 18-70 years with a diagnosis of Chronic Kidney Disease in SUSALUD. The estimation of the glomerular filtration rate by the MDRD formula was done using the National Kidney Foundation calculator. An analysis of linear correlation was conducted and matching was explored by means of the Kappa statistic.


Results: 53% of the studied group was female and the average age was 56.3 years. The average clearance in urine 24 hour volume was 69.3±24.2 mL/min and the average clearance by MDRD was 62.3±17.5 mL/min. The correlation between the two tests was moderate (57.8%) and the matching force was weak (0.25)


Conclusions: the weak concordance between the urine 24-hour volume creatinine clearance and the MDRD formula in this group of patients does not allow for recommending its use. Due to the difficulties in collecting and interpreting creatinine clearance, new studies are required to recommend the use of the MDRD formula in our population.
