Hepatic trauma over a ten-year period. Analysis of 1675 cases

Abstract
Retrospective review of 1.675 patients suffering hepatic trauma was done. These patients were admitted to the Emergency Service of Saint Vicent University Hospital from 1970 to 1980. 1.097 patients presented stab wounded injuries (65%); 412 patients had shot gun injuries (25%); and 134 patients were car accidents. In 1.578 patients (94%), the hepatic lesions were managed by suture and drainage. Hepatic lobectomy was carried out in 97 patients (6%). Morbidity rate was about 15% and it was higher among close trauma patients (23%). Associated lesions were more frequent in stab wounded patients; since they were brought in shock condition they were bound to receive more blood transfusions and they were due to remain longer at the hospital. External biliary fistulas were present in 11 patients (0.5%). lntracaval by pass, was used in massive hepatic trauma or porta hepatic vascular injury; 6 of these patients did survive.
